Hi there,

I am a Home Share provider and recently received a Health profile form in the mail and I am aware it is a silly question but do I fill out the Health information of the person in my care or that of my own?

I love the online forms. After years of begging CLBC to get forms online and eliminate the paper work for caregivers, it is encouraging to see your company provide that service.
